You will need to submit a clear ID style photo with a white background. Here are instructions on the type of image needed. https://mmcc.maryland.gov/pages/patient-id-cards.aspx

You will also need to provide (2) Proof of Address ONLY IF your ID does not have a valid address listed. Here are instructions to help you provide the correct information.
